If you are new to Capital District Colon & Rectal Surgery Associates, P.C., and would like to download and fill out our Patient Information forms in advance of your visit, you may do so here.  Please fill out the Patient Information form, then the Patient Data form, and finally, sign the Signature on File form and HIPAA form.  Get the forms here. 

If you are undergoing a Colonoscopy procedure and have lost your Preparation form, you may view either the Miralax or Fleet Enema Preparation forms here.
